Pancrelipase is a prescription enzyme mix used when the pancreas can’t make enough digestive enzymes. If you have cystic fibrosis, chronic pancreatitis, pancreatic surgery, or other causes of exocrine pancreatic insufficiency (EPI), pancrelipase helps your body break down fats, proteins, and carbs so you get nutrients and avoid weight loss and diarrhea.
How pancrelipase works and when to take it
Pancrelipase contains lipase, amylase, and protease — enzymes that mimic natural pancreatic juices. You take it with every meal and most snacks. Swallow the capsules whole with a sip of soft food or water while eating. For people who can’t swallow capsules, some preparations can be opened onto soft acidic food (like applesauce) — follow pharmacy instructions exactly so enzymes stay active.
Dosing is individualized. Doctors base the dose on body weight, the fat content of your meals, and how well symptoms improve. The goal is to reduce steatorrhea (fatty stools), normalize stool frequency, and support weight gain or maintenance. Your provider will adjust dose gradually until symptoms are controlled.

Common side effects are mild: stomach cramps, nausea, or constipation. Rare but serious problems include allergic reactions or fibrosing colonopathy in very high-dose use, mostly reported in young children with cystic fibrosis when doses exceeded recommended limits. Always follow your prescriber’s dosing and check with them if symptoms worsen or new problems appear.
Drug interactions are uncommon, but pancrelipase can affect absorption of some oral drugs. If you take iron, certain diabetes medicines, or other long-term drugs, mention pancrelipase during medication reviews. Also tell your provider about any history of food allergies if you will open capsules onto food.
Storage is simple: keep capsules in a cool, dry place and follow the expiration date. Don’t store them in direct sunlight or a bathroom. If you miss a dose during a meal, take it as soon as you remember with the rest of the food. Do not double up later to make up for missed doses without medical advice.
Finally, track outcomes: weight, stool quality, and appetite are good signals the therapy is working. If you still have greasy stools, persistent pain, or ongoing weight loss, contact your healthcare team — adjustments or tests may be needed.
